Please complete the information below so our team can set up your client profile, billing details, authorized contacts, delivery preferences, compliance requirements, and recurring rental needs.

This form is not for one-time event quote requests. If you need pricing for a specific event, please submit an event rental request instead.

Submission of this form does not create an open credit account, guarantee billing terms, or modify Funtastic Rentals’ rental agreement. All rentals remain subject to availability, a signed rental agreement, approved payment terms, delivery access, and final approval by Funtastic Rentals.
